Connect Me for People with Hypertension
It is important for good health that your blood pressure is a ‘normal level’. This will reduce the risk of damage to your body, from things such as strokes and heart disease.
Monitoring your blood pressure at home gives a more accurate blood pressure reading, especially for those people whose blood pressure can be raised during visits to the practice.
Connect Me allows you and your health provider (doctors, nurses and pharmacist) to monitor and manage your blood pressure without needing to attend the practice for regular reviews. It is a simple, convenient and free service.
Connect Me is a Scottish Government programme within the NHS which focuses on improving healthcare delivery at home. It involves you checking your blood pressure at home at set times and days chosen by yourself. A message will be sent to you either by email, text or automated phone call prompting you to check your blood pressure and instructions on sending in the reading.
Your blood pressure readings will be collected and forwarded to us on regular intervals so that they can be monitored and reviewed.
To register for Connect Me please make an appointment with the Practice Nurse. Please note, you will be required to supply your own blood pressure monitor.
