Patients' Rights and Responsibilities
THE PRACTICE |
PATIENTS |
Patients will be treated with courtesy, respect and confidentiality at all times | Patients have responsibility to treat our staff with equal courtesy and respect at all times |
Urgent problems will be seen the same day by whichever doctor is available | Only request an urgent appointment when you feel it medically necessary and not safe to wait |
The practice will monitor the waiting times for appointments and inform you of any significant delays | If you cannot keep your appointment please let us know as soon as possible so that we can give that appointment to another patient |
Wherever possible you will be seen as near to your appointment time as possible | Please try to be flexible when making your appointment to allow yourself to see the doctor of your choice |
Medical staff will visit you at home if you are too ill to come to the surgery | Please do not ask for a home visit unless it is absolutely necessary. Home visits should ONLY BE for the very ill/housebound |
The practice will inform you of the services it provides and how you can access them. | Please understand that each appointment is for 10 minutes (emergency slots are 5 minutes) |
Advice will be given and/or medication prescribed to patients as required. Repeat prescriptions will be processed within 48 hours. You will be asked to see the doctor to have your medication reviewed at least annually. |
Please remember a prescription may not always be necessary. Order repeat prescriptions before your medication runs out Order your prescription using the dedicated phone line between 2-3, by posting your request slip in the box in reception or by royal mail |
Patients will be removed from our practice list as a last resort if the practice/patient relationship has broken down irretrievably. Patients who continually fail to keep appointments may be removed from the list. |
Abusive or violent behaviour will not be tolerated and will result in removal from our list. If you are unhappy with our service please tell a member of staff and we will try to resolve the problem.
