Long term condition appointments
Mackenzie Medical Centre is changing the way we care for our patients with long term conditions. Long-term conditions (LTCs) can be defined as any ongoing, long term or recurring conditions that can have a significant impact on people’s lives. (For example, diabetes, asthma, COPD, hypertension, heart disease). Patients are invited in on or around their birthday month for an annual review of their condition(s) and medication. We will now be inviting patients in for review by text message. If you do not have a mobile phone, you will receive your usual yellow letter invitation. The various types of review appointments are structured differently, depending on the condition being reviewed. We want to put you in control of your health by ensuring you have up to date information and advice tailored to you.
Our new appointment process consists of three stages.

Pre-appointment questionnaire sent via text

Appointment with the Practice Phlebotomist for a blood test (if required)

Long-term condition review with Senior Nurse Julie. This should be 1 week after your blood test so that your results can be reviewed beforehand
